Guidelines for Child Bereavement Practitoners

This assessment book has been developed to offer child bereavement practitioners a model on which to base their own practice.

It forms the paper-based part of the assessment tools we use at Winston's Wish to ensure that we can evaluate the impact:-

• of a particular death
• on a particular family
• at a particular time
• living in a particular community.

The contents have been developed over many years and are built on the experience of many practitioners both within and beyond Winston's Wish.

This pack contains the assessment form with suggested prompt questions to elicit the information on which you can base your own assessment of a family's needs.  The assessment form assumes a level of expertise in conducting family assessments.
