Guidelines for Child Bereavement Practitioners

These forms have been developed to offer child bereavement practitioners a model on which to base their own practice. The forms contain suggested prompt questions to elicit the information on which to base an assessment of a family’s needs – determining the impact of this particular death, on this particular family, at this particular time, living in this particular community. The ‘Family Assessment’ form covers the assessment of the parent/carer and one child (as well as questions aimed at, for example, the child’s teacher(s) or other concerned professionals). The ‘Child/Young person’ form is for an additional child. They are sold together as experience shows that most family assessments include 2 children.
